Most referrals are done electronically over the internet. However, we do need the appropriate information in order to complete these. Our office policy requires an appointment for a new issue requiring a referral.
Information which is needed to process a referral includes specialist or provider's name and ID number, your insurance ID number, specialist's phone number, date and reason for appointment.
Local Pharmacies: Refills for current medications are usually sent to the pharmacy electronically. You may be asked to come in for an appointment prior to the refill based upon the prescription and situation. Controlled substances can not and will not be refilled in this manner.
Mail Away Pharmacy Information: Mail away pharmacies are quite difficult for us to deal with. Therefore, it is our office policy that if you need a refill from a mail away pharmacy, you will need to bring the form into the office. Only bar coded forms will be faxed. Please do not call the pharmacy to have them fax us a refill form, because this will only delay your prescription. The quickest and easiest way to have prescriptions refilled through a mail order pharmacy is to pick up a hand written prescription at the office.
